Sizes all wrong in Slic3r??

Please have a look at the attached image and see what problem I'm having.

Why is this happening? Is it a common Slic3r problem?

I really need the inside measurement you see in the picture to be fairly accurate, would anyone have any advice on how to fix it?

On the outside measurement it's not too far out, is this acceptable or can I get it better?
