Print Pause and Printer Stopped working

ps: I have done Print Pause/Resume many times before and it was working fine but this time something went wrong.

Few days back I was printing this [www.thingiverse.com] spool holder. Print was going fine but till 1:30 hr (total time 2:15), later noticed the it was getting loose from the bed. Thought let's pause the print and stick some kapton on brim, see the picture

Unfortunately I could not resume it not sure why but I saw
- printer disconnected message
- Usb driver error message

Since than I did stop/start of pronterface as well as my laptop many time but printer is not working. pronterface is connecting to printer but

- axis are not moving
- hotend is not heating but bed is heating fine though sensor reading is not coming to UI
- temperature shown by pronterface is always T:0 B:0

I checked few things...

Polu Driver 4988
- E and Z are hot while X and Y are slightly less hot ( this was after printer was started and s/w was connected but without any success with moving axis), voltage reading on polu are:
- VMOT/GND: 19.47v
- VDD/GND: 4.47v
- VREF/GND: 0.4 to 0.42v
- EN/GND: 0.85

I was discussing the issue with Kliment on IRC, he wasn't happy with VDD/GND reading said it must be 5v and my laptop might be an issue. However please not this is the only laptop i have and everything was working fine till few days back. Today I checked the V coming on USB and it is 5V.

From the following log of commands fired from pronterface, it seems the s/w is able to communicate with the printer guess there is not issue with USB driver or comm link between PC and Printer. Note I have already re-installed the USB driver.
