Slicing with Slic3r and getting a good part fit

I am currently printing PLA at 195C with an E3d v6 hotend, with 1.75 mm filament and 0.4 mm nozzle and slicing with the most recent version of Slic3r. What I have found is that after calibrating X,Y,Z, and the extruder, when I print either a calibration cube or nickel holder, the X,Y, and Z are quite good but the internal X and Y are off by at least 2%. After changing the extrusion multiplier to ~0.9 and forcing the perimeter to 0.6, then the internal X and Y are okay. The quality of the print though has gone down since the fill is not 100% on the top or bottom. I would like to be printing at at least 0.2 mm layer thickness. Any suggestion to changes in the config settings.

This is a much-discussed issue. Perhaps the best discussion of it is in nophead's blog from February 2011.
