Hot end cooling fan

I got this i3 with no parts list and or directions. It is a dual extruder with a inductive sensor. It's together with a whole lot of youtube and Google search. I'm look for verification that I wired it up right for the hot end, fan and the inductive sensor. D9 is double tap with the #2 hot end and fan. Inductive sensor replaced the Z end stop in port 5 and The single red wire is taped in the 5v+ port. These are the two things that I couldn't find a definate answer on.

What controller are you using? RAMPS? pictures please
