Minor RepRapFirmware release 1.15d

I've just released this at [github.com]. The changes since 1.15c are:

* Reduce incidence of false heating fault alarms, in particular "temperature rising much more slowly than the expected 0.0C"
* On the Duet WiFi, stepper drivers beyond the first 5 are now assumed not to be TMC2660 drivers, so the Enable signal is used as a driver enable instead of a chip select

The recommended version of DuetWebControl is still 1.12.

Large delta printer [miscsolutions.wordpress.com], E3D tool changer, Robotdigg SCARA printer, Crane Quad and Ormerod

Disclosure: I design Duet electronics and work on RepRapFirmware, [duet3d.com].

Upgraded without any issues and did two test prints from cold and so far the bed has not caused an error. One thing I did notice and this may be a coincidence as I'm printing a new part but the printer seems to be running smoother.
