Printing issues ...
You could reverse the stepper and adjust the X-axis opto flag, but then you would have a Mendel that printed with an inverted X-axis. That could be fixed in software, not sure how difficult it would be or if the options you need are readily available. Or you could hack the firmware to home with X high and to consider that home position to be whatever coordinate puts zero X roughly where it should

The stock firmware won't work with the MAX6675 either, and reading from it is much more complex, so unless you have had some success with this chip in the past you would be best off just buying a replacement AD595 which can be read with one, simple, line of code.

Im looking at the extruder 2.2 schematics and the techzone remix diagrams. It looks like the way forward might be to connect the thermocouple board to one of the 'extra' headers and then rewrite the firmware to read from that header. Using the thermistor header for this requires modifying the board.
Looks easy to rewrite, line 67 in extruder.h is where the temperature is read - and looks broken

It looks like the reprap software is failing because it finds a reprap, talks to it, and gets garbage back. Post a shot of the errors you get with the firmware upload.

Google the 'stk500_getsync' error messages, you should come across some threads which discuss various solutions. I can't see anything that looks like The Answer, but there are a lot of suggestions out there.

Ive seen mention of a bug which requires you to press the reset button while uploading. Naturally I can't find the post right now, but it is mentioned in the wiki. It was supposedly fixed on the version 2.2 extruder boards.

Now you have a problem. If you look in temperature.h, you'll see that NUMTEMPS is defined within
#ifdef USE_THERMISTOR
which is undefined because you have defined AD595-THERMOCOUPLE instead. Theres no equivalent section within temperature.h. I'm not sure how this is meant to work.

Would it be possible to cast an aluminium block around the nozzle? Google suggests that the melting points make it possible, but I don't know about the thermal stresses involved or if aluminium's thermal expansion would be enough to cause problems.
If it can be done, then you would have a heater block with excellent contact with the nozzle and which could be placed right at the tip.

Set extruder count to one.
The 'mainly for darwin' part refers to the settings for motherboard == 1 - everything from the 'if motherboard == 1' line to the '#endif' line is only compiled when motherboard is set to 1. You want to be playing with the settings in the next section down.

Comment with a pair of forward slashes as seen throughout the file. It shouldn't make a difference, but its easier to remove a comment marker than to look up what was originally in the file.

I get similar errors if I forget to tell it to use an Arduino Mega and set 'motherboard' to 3. Make the appropriate settings and, if needed, install the sanguino files before trying to compile.

Install something like ConText or any decent text editor and it will be able to work with linux style plain text, which uses a different character to indicate newlines.
While your at it, disabled the 'hide extensions for known file types' feature in windows, it can make things really confusing.
