HELP! Slic3r generates wrong G codes for my model confused smiley

I've made simple stl file with 3 holes, and now processed with Slic3r but it doesn't work correctly.

Please see attached picture for this.
The side holes are gone.

Most of the models I've made were able to be processed correctly,
but sometimes, like this, Slic3r goes mad.

What's going on with this?
Please let me know whatever you think of.
